Policies:

Introduction:

These policies are effective as of February 2023, & are subject to change.

As math specialists, we have your child’s best interest in mind as we created these policies. Please trust the process and support us in carrying out these policies so we can help your child make the most progress while working with us.

Custodial Rights

I have custodial rights for this child and can provide documentation.

Parental/Guardian Consent

I am consenting to have my child receive multisensory math services online.

MFM Informal Assessment

I understand that the MFM assessment is informal in nature and I will be given an informal report of my student’s starting point.

Length of Assessment

I understand that the informal assessment can take up to 8 sessions to complete with my child’s assigned specialist.

Session Length

I understand that sessions are either 25 or 55 min in length depending on the selection of my Tier plan.

Student Materials

All materials for lessons will be mailed before starting sessions. The cost of materials is included in your monthly fee. Note, if you opt to leave Made for Math prior to the five-month mark, you are responsible for returning math materials. Failure to do so will result in a $150 fee.

Financial Responsibility

I accept full financial responsibility for these services.

Flat-Rate Service

I understand that MFM is a flat-rate monthly service. I can stop at the end of the billing cycle. Unused lessons in the prior month will not be refunded. The average session count will fluctuate to accommodate holidays and vacations.

Credit Card/Debit Card Fees

I understand that I will automatically be charged a 3% fee if I elect to use a credit or debit card for our MFM tuition.

ACH/eCheck will be free of any fees.

Missed Payment/Returned Checks

I understand that missed payments will stop services until payment is made. Notification via email/text will be made if this happens.

I will have one week to rectify the payment. Failure to do so could cause the forfeit of my spot. In the case of a returned check, a $30 fee will be applied to my account.

No Bill of Insurance

I understand that insurance does not cover these services.

Student Schedule Change Fee (After semester calendar has been submitted)

I understand that if I need to change the regularly scheduled time with my math specialist, a $50 change fee may be incurred.

*This does not apply to rescheduling due to illness, piano recitals, etc., only an ongoing schedule change.

Lesson Calendar

We schedule lessons 4 months in advance. We will contact you to make arrangements to schedule with your math specialist. Note that the month of December is always pro-rated to half of the price of your currently selected tier package.

Rescheduling/No Show

If my student fails to show up for a lesson without a 24 hr. advanced notification, the lesson will not be refundable or available for rescheduling. In the event that my student is late to a session, teachers will only wait for 15 minutes. If after 15 minutes my student still has not arrived the session will be considered a “No-show session” and is not eligible for rescheduling. I understand that teachers will not return to finish a session after that 15-minute grace period.

Life happens and we understand that. If you need to reschedule a session, please reach out to your math specialist at least 24 hours in advance to find a time that works for both of you. We do allow 2 reschedules per month, but after that, we reserve the right to refuse the rescheduling.

No Homework Help

I understand that MFM is an intervention service and will be filling gaps in my child’s understanding, not helping them navigate current math classes or tutoring them on their homework. We will strive to align our work with schoolwork when possible.

No MFM Homework Creation

I understand that MFM is an intervention service and will be using repetition during sessions to aid memory.

No Grades

I understand that MFM will not be providing grades to report to schools or to various states for homeschooled students.

No Teacher Liaison/Communication

I understand that our math specialist will not be communicating with my child’s teacher or teaching them in MFM’s methods. If communication needs to happen, I understand that I am in charge of initiating contact with the teacher.

You are welcome to forward weekly updates from your math specialist.

No Sessions at School

I understand that our math sessions will not be held in a school setting.

Math Specialist/Parent Communication

I will respond to email/text/phone calls from MFM within 24 – 48 business hours.

Math Specialist/Student Communication

I understand that swearing or speaking disrespectfully to teachers is not allowed in sessions. If this happens, a teacher has the right to leave the session. The parent will be notified, and the session will not be refunded. If the problem persists, MFM has the right to terminate the student contract.

Student Participation

I understand that students are expected to be active participants in sessions and listen to directions given by their specialists. If my child refuses to participate and/or follow directions, the specialist reserves the right to end the session. The parent will be notified, and the session will not be refunded. If this is a consistent problem, and the parent and specialist are unable to resolve the situation together, then MFM reserves the right to cancel the student contract.

Lesson Recording

I agree to have each lesson recorded.

Use of Recordings

I understand that we do use parts/pieces of lessons for marketing and training purposes. MFM will always notify me before using video for marketing purposes.

Student Preparedness

I understand that my student should participate in lessons while seated at a table or desk in a quiet space. I also understand that my child should have their materials ready for the lesson and should leave his or her cell phone in another room.

Student Tech Needs

I understand that my child needs to come to the session with earbuds, a computer, and a mouse. The camera and microphone must be turned on.

Devices

I understand that we cannot use a Chromebook, iPhone, iPad, or smart device for lessons at MFM. The only approved devices are desktop and laptop computers.

Final Parent/Guardian Consent

By typing your name below, you are agreeing you understand all of the above terms of service and policies from Made for Math. Failure to do so may forfeit your admittance and continued access to services.

Privacy Policy

Protecting your private information is our priority. This Statement of Privacy applies to https://madeformath.com, and Tutor House, LLC and governs data collection and usage. For the purposes of this Privacy Policy, unless otherwise noted, all references to Tutor House, LLC include https://madeformath.com and Made for Math. The Made for Math website is a services and information site. By using the Made for Math website, you consent to the data practices described in this statement.

Collection of your Personal Information

In order to better provide you with products and services offered, Made for Math may collect personally identifiable information, such as your:

– Job Title

If you purchase Made for Math’s products and services, we collect billing and credit card information. This information is used to complete the purchase transaction.

We do not collect any personal information about you unless you voluntarily provide it to us. However, you may be required to provide certain personal information to us when you elect to use certain products or services. These may include: (a) registering for an account; (b) entering a sweepstakes or contest sponsored by us or one of our partners; (c) signing up for special offers from selected third parties; (d) sending us an email message; (e) submitting your credit card or other payment information when ordering and purchasing products and services. To wit, we will use your information for, but not limited to, communicating with you in relation to services and/or products you have requested from us. We also may gather additional personal or non-personal information in the future.

Made for Math collects and uses your personal information to operate and deliver the services you have requested.

Made for Math may also use your personally identifiable information to inform you of other products or services available from Made for Math and its affiliates.

Sharing Information with Third Parties

Made for Math does not sell, rent or lease its customer lists to third parties.

Made for Math may share data with trusted partners to help perform statistical analysis, send you email or postal mail, provide customer support, or arrange for deliveries. All such third parties are prohibited from using your personal information except to provide these services to Made for Math, and they are required to maintain the confidentiality of your information.

Made for Math may disclose your personal information, without notice, if required to do so by law or in the good faith belief that such action is necessary to: (a) conform to the edicts of the law or comply with legal process served on Made for Math or the site; (b) protect and defend the rights or property of Made for Math; and/or (c) act under exigent circumstances to protect the personal safety of users of Made for Math, or the public.

Tracking User Behavior

Made for Math may keep track of the websites and pages our users visit within Made for Math, in order to determine what Made for Math services are the most popular. This data is used to deliver customized content and advertising within Made for Math to customers whose behavior indicates that they are interested in a particular subject area.

Automatically Collected Information

Information about your computer hardware and software may be automatically collected by Made for Math. This information can include: your IP address, browser type, domain names, access times and referring website addresses. This information is used for the operation of the service, to maintain quality of the service, and to provide general statistics regarding use of the Made for Math website.

Use of Cookies

The Made for Math website may use “cookies” to help you personalize your online experience. A cookie is a text file that is placed on your hard disk by a web page server. Right to Deletion

Subject to certain exceptions set out below, on receipt of a verifiable request from you, we will:

Delete your personal information from our records; and
Direct any service providers to delete your personal information from their records.

Please note that we may not be able to comply with requests to delete your personal information if it is necessary to:

Children Under Thirteen

Made for Math does not knowingly collect personally identifiable information from children under the age of thirteen. If you are under the age of thirteen, you must ask your parent or guardian for permission to use this website.

E-mail Communications

From time to time, Made for Math may contact you via email for the purpose of providing announcements, promotional offers, alerts, confirmations, surveys, and/or other general communication. In order to improve our Services, we may receive a notification when you open an email from Made for Math or click on a link therein.

Made for Math reserves the right to change this Privacy Policy from time to time.
