MFM Informal Assessment

Length of Assessment

I understand that the informal assessment can take up to 8 sessions to complete with my child’s assigned specialist.

Student Dismissal

I understand that my child will be dismissed from class 5 minutes before the end of the session.  The last 5 minutes of the session are reserved for the teacher to download the Zoom video and upload it securely to the drive, as well as take notes on student progress for optimal planning and customization of student lessons.

All materials for lessons will be mailed before starting sessions. The materials cost will be covered in our one-time setup fee ($400) after your child has been placed with a specialist.

Lost materials will not be replaced. I can request a new set for an additional fee of $300.00.

I understand that if I need to change the regularly scheduled time with my math specialist, a $50 change fee may be incurred.

*This does not apply to rescheduling due to illness, piano recitals, etc., only an ongoing schedule change.

Lesson Calendar (After semester calendar has been submitted)

I have selected my lesson dates on the calendar and understand the cost is the same each month, regardless of how many lessons fall in that month.

No Grades

I understand that MFM will not be providing grades to report to schools or to various states for homeschooled students.
